Interdisciplinary Medical Team
The Care Plan for each individual on the Transitional Care Unit (TCU) will be coordinated by an experienced and professional staff, including the primary care physician, registered and licensed practical nurses, registered and licensed therapists, the TCU Nurse Case Manager, Social Service Director, and the TCU Orthopedic Medical Review. The medical team will work in conjunction with the patient and family members.
Continental Nursing and Rehabilitation Center offers a wide variety of sub-acute rehabilitation services up to seven days a week including physical, occupational, and speech therapy. Our full-time, experienced therapy staff members- which include registered licensed physical and occupational therapists, rehabilitation aides, and a speech language pathologist- are specialists in their area of expertise. Our facility includes state-of-the-art Nautilus equipment for physical therapy. Our dedicated and experienced rehabilitation department will work closely with the patient's primary care physician to complete a comprehensive assessment and formulate an aggressive, proactive and individualized therapy plan which will enable the patient to regain his or her highest level of independence.
The TCU Nurse Case Manager acts as the liaison between the hospital and other medical professionals, the insurance company and family members. The Nurse Case Manager will go to the hospital to assess each patient, gather the necessary medical information needed for admission, and answer any questions the patient may have about our facility. Any special needs or concerns prior to admission will be addressed by the Nurse Case Manager. Prior to discharge from the facility and returning home, the Nurse Case Manager and the Social Service Director will work closely with the patient and family members to determine the needs of the patient, whether it be medical equipment or home health services.

Continental Nursing and Rehabilitation Center welcomes visitors, and the staff is ready to accommodate any special requests. We can help coordinate a family gathering for a birthday or anniversary or arrange to have lunch with visitor's loved one.

The Transitional Care Unit (TCU) at Continental Nursing and Rehabilitation Center has been thoughtfully designed to offer a comfortable, homelike environment for individuals coming from the hospital or short-term rehabilitation.
Our warm, spacious rooms have complementary in-house television with remote controls, direct dial private telephones, daily newspaper delivery, private refrigerators and personal laundry service.
The goal of the TCU is to facilitate a smooth transition form the hospital after a surgical procedure or stroke to home by means of intensive rehabilitation services.
The TCU may also be appropriate for patients recovering from a prolonged or newly diagnosed illness who need rehabilitation as well as 24-hour skilled nursing care.
Discharge Planning begins the day you walk in the door. The Social Service Director and TCU Nurse Case Manager will work closely with the nursing staff, physician, patient and family members to determine a discharge date and what services the patient might need at home by doing an assessment of the prior home environment.
